In order to create a table of contents manually in FineReader PDF 15 you may use a feature for Linking to a specific text fragment in the current document:

1. Make sure that the document is recognized. 
2. Open it in PDF Editor; 
3. Select the text you would like to link to a specific page/place in the file.;
4. Go to Edit > Add Link; 
5. Specify a page number in this document to the corresponding field:
   
6. Click on Ok.
