Documents consists of checkmark but it was representing as "Radio Button", in FLS those radio button area was captured as region and in Blocks it was linked to a Checkmark block. Once it was imported in Administration Station the checkmark type was made to "Square".
When images are loaded it was not recognized properly considering the radio button circle as marked field. There is any way to recognize the radio button as checkmark.

I believe your setup is almost ready, but make sure you:
1) use a blank form for your document definition image, without any filled in checkmarks
2) change the checkmark block from Square to Auto in the Recognition tab of the checkmark block's Properties
Test it and it should recognize properly.
